If you are going to leave your child with me, I need to be in a position to deal with any emergencies which might come up. I’ve had my school since 1980, but tomorrow I might not be able to say that there has never been a medical emergency at my school. Fill in and give me this Registration Form. We both want to make sure your child is taken care of.

I also need to know whom to contact about billing and how to let you know if I have to cancel a class. I charge $25 one time for a family to register and be entered into my system.
